Menopause Guide

What is the menopause ? back to top

The menopause is sometimes called "the change", and shows itself by the ending of menstruation. It happens at an average age of about 47 to 52. but can happen earlier or later.

A premature menopause can start for different reasons for instance if the ovaries have to be removed or a hysterectomy.

The menopause means the end of egg production by the ovaries but, menstrual cycles can still occur without ovulation, as the menopause approaches. This is why pregnancy is very rare after the age of 50. A woman's periods do not usually stop suddenly, but sometimes this can happen. The main effect of the menopause is reduced production of oestrogen.

Symptoms of menopause back to top
Every woman experiences the menopause differently. Some hardly notice the change just perhaps irregular periods. But other women suffer with all the symptoms. The most common symptoms of the menopause are hot flushes these vary from woman to women in intensity. Also night, insomnia, headaches, mod swings and weight gain.

Psychological changes
Depression, stress, mood swings, tiredness, headaches. Forgetfulness or irritability can be distressing for both you and your friends or family.
Hot flushes and sweating
The most common symptoms by far are ‘hot flushes’ and sweating attacks. These episodes can happen at any time, as often as several times an hour.

Sleep disturbance
Sleeping difficulty can be due to restlessness or night-time sweats. Some women can sweat heavily and have to get up to change the sheets a few times a night
Physical changes
Skin becomes thinner during the menopause
The vagina doesn't produce as much lubrication

There is a gradual rise in the risk of heart disease and stroke after the menopause.
Falling oestrogen levels result in changes in cholesterol and fat levels in the blood

Sex may be painful or a lack of interest in sex, and orgasms can become less intense.
The bladder may need to pass water more often.
Osteoporosis
Because of the drop in oestrogen, women tend to lose bone mass and strength for several years following the menopause. This can make the bones more fragile.

Menopause HRT Treatment back to top

HRT or Hormone Replacement Therapy either oestrogen only or a combination of progesterone and oestrogen is used to reduce menopausal symptoms.
There are different ways of taking HRT, the most usual being a daily tablet alternatives include skin patches, implant under the skin, a gel applied daily to the skin, a ring inserted into the vagina, or a nasal spray.
The majority of women have no side effects with HRT but the following are fairly common:
nausea, breast tenderness, weight gain, fluid retention.

HRT is effective at relieving hot flushes and vaginal dryness and can improve  general sense of wellbeing. However The long-term benefits of HRT have recently been brought into question.

Any woman considering HRT should discuss the risks and benefits with her doctor before making a decision about treatment.

Soy and menopause back to top

Many researchers believe that Japanese women have a lower incidence of hot flushes and other symptoms because of their soy-based diets.
In the Asian population a delayed menopause is observed and associated symptoms are attenuated compared to the Western population; this has been related to the presence of soy isoflavones in the Asian daily diet (their average intake is of 45 mg isoflavones/day, compared to 5 mg isoflavones/day in the Western population).

Epidemiologic studies also relate these compounds to the prevention of hormone dependent cancer and to the low morbi-mortality percentage of cardiovascular origin and osteoporosis among the Asian population.
